Plaza del Rio Care Center is a 128 bed skilled nursing community located on The Freedom Plaza Retirement Community Campus in Peoria, Arizona. Whether a resident’s needs are for short-term or long-term care, our skilled nursing services are designed to promote comfort and security in a high-quality and compassionate environment.
As residents’ conditions progress and they move through different care levels, our staff works with their families and their physicians to transition them to the most appropriate setting whether that is back home, or to an Independent Living Community Assisted Living or Memory Enhancement. Each resident’s care is customized for their specific needs and preferences and delivered around-the-clock by trained professionals.
Skilled care frequently begins with a comprehensive initial assessment and interdisciplinary conference including discharge planning. Our licensed professionals are available to answer questions and address the concerns of prospective residents or their families.
Common reasons why seniors may need skilled nursing care include:
- Rehabilitation care due to fractures or joint replacements
- General wound care provided by licensed nurses certified in advanced techniques for treatment of complex wounds.
- Acute medical conditions
- Disease Management including rehabilitation and teaching for conditions such as Congestive Heart Failure, Diabetes, Stroke, Parkinson's Disease and others.
- Interim medical care following a hospital stay
- Terminal illness care (in cooperation with hospice)

Plaza del Rio Care Center also contracts with many Insurance Companies providing short term rehabilitative services.
